Subjective data

Joi Anderson complains of pain in the bilateral thigh and hips. She says that,, the pain has lasted for a day and it is continuously increasing. Additionally, Joi denies that she has no trauma and has not had excessive exercise. Joi also says that she feels exhausted all the time and has breathing problems. She also has problems with going for short calls.

Objective data

Joi Anderson was diagnosed with sickle cell anemia, when she was 4 years old. The patient has no known allergies. From the laboratory tests, it was noticed that the pain felt by Joi on the bilateral thigh and hips was brought about by a urinary tract infection. The urinary tract infection is evidenced by increased urinary frequency and dysuria and high white blood cells (WBC) count. Importantly, the low Hemoglobin level (Hb) is pointing to sickle cell anemia.

Other assessment data I may need to collect is about dysuria. With regard to dysuria, I would conduct laboratory tests to establish if it is the cause of pain and discomfort during urination. Additionally, I would ask Joi if her urine has foul smell and has blood which are major symptoms of dysuria. I would also, conduct tests aimed at establishing the cause of breathing problems.
